How To Get a Medical Cannabis Document
Follow these 5 easy steps to obtain your medical cannabis document. Please ensure to read through all 5 steps first before you begin.

1: Create Your Mendo Account
If you have not already done so, use this button to register and create a Mendo account. If you already have a Mendo account, you can move on to step 2.
2: Click On The Medical Document Form
Ensure you have your Health Card ready for Step 3.
3: Complete The Medical Document Form
With your Health Card ready, enter the required information into the medical document intake form. When you reach the appointment calendar portion of the form, move on to Step 4.
4: Schedule Your Online Consultation
The last page of the Medical Document Intake Form contains a calendar where you can schedule your online appointment. Once you’ve completed your consultation with a physician and have been verified, you can move on to Step 5.
5: Place Your First Order
Once the process is complete and you have been verified, you can place your first order!
